Admission of the candidate to various P.G. Degree/Diploma course will be given by the Principal / Dean of the respective Medical College or Medical / Dental faculty of CSM Medical University, Lucknow as the case may be. |

A candidate who has been allotted a P.G. Degree / Diploma course in a particular Medical / Dental College shall have to complete all the admission formalities including depositing the prescribed fees, by the date notified for this purpose by the respective Principal / Dean of Medical College or CSM Medical University, Lucknow as the case may be. Those who fail to do so will loose their claim for admission. |

Admission shall be given only after the candidate produces, in original and one photocopy each of the following documents at the office of the Principal / Dean of the Medical College concerned or the Registrar, CSM Medical University, Lucknow, as the case may be |
(a) |
High School Certificate as proof of date of birth. |
(b) |
Mark sheets of all professional examinations. |
(c) |
MBBS / BDS passing out certificate or degree. |
(d) |
Compulsory Rotatory Internship completion certificate. |
(e) |
Permanent Registration Certificate (U.P. State / MCI / DCI). |
(f) |
Employers relieving certificate, if employed. |
(g) |
S.C. / S.T. / O.B.C. / Physically handicapped certificate, if applicable. |
(h) |
Allotment letter, issued by Chairman of the Counselling Board of UPPGMEE - 2010. |
(i) |
Admit Card of UPPGMEE - 2010. |

Candidates who have passed MBBS from Himalayan Institute of Medical Sciences, Dehradun are required to submit a domicile certificate of U.P. on the prescribed proforma (Annexure 6) only and a certificate from the Principal / Dean of being admitted in the year 2000 only. |

After the admission process is completed the candidate will submit in writing his / her joining report to the Head of the Department concerned. |

The identity of the student will be reestablished on the basis of his / her photograph and his / her finger prints after his admission. He should make sure that he is available for this checking. If he / she is not available, this would be counted as unfair means and his candidature will be cancelled. If in the checking, the identity of the candidate does not match with that of the identity established at the time of taking examination it will be treated as unfair means and the candidature of the candidate will summarily cancelled. |
