 All India DM/ MCh Entrance Examination 2008
 
 
There are two application procedures viz
a

Online application by Internet

b

Application by paper format

Online Application procedure

It is advisable to apply online using Internet, this would also allow you to get a admit card online itself.
Online application can be made on website www.kgmcindia.edu
You will be required to fill in the number of the Bank Demand Draft in your online application. Hence you should have a demand draft ready before you proceed for the online application. The Bank draft should be of Rs 1200/- if you are applying for one subject or of Rs 1500/- if you are applying for 2 subjects.
All candidates opting for online application are required to submit to the “Coordinator, All India DM/MCh Entrance Examination” a printout of their application together with photograph, certificates and Bank Draft by post/speed post or hand (but no courier). Candidates in service should send this print out through their employer.
You will hence need a printer to print out the online application form; this printed form should be received by the Coordinator on or before the last date 7 June 2008.
You should Directly upload your photograph online itself; alternatively if you are unable to upload your photograph you can paste a passport size photograph on the printout of your application at the designated place
After you have submitted your application online, note down your username and password for future login. Login will be required for obtaining the Admit Card. You can also print out your application after login at a later date
Print out the application, paste the photograph, attach the demand draft (write your name and registration ID behind the demand draft) and required copies of certificates and send to the Coordinator, All India DM/MCh Entrance Examination 2008, PHI Building, C.S.M. Medical University, Lucknow, so as to reach there by 4 PM of June 7, 2008
Please note that your online application procedure is not complete till your application has been received at the Coordinator office along with Bank Demand Draft by post/speed post/hand. Couriers are not acceptable.
Admit card shall be available online using your username and password from June 14, 2008. Admit card shall not be posted to any candidate
Application by paper format
Please be advised that admit card shall not be posted to any candidate and candidates choosing the paper format option (ie not applying online) shall have to personally collect the admit card from the Office of the Coordinator All India DM/MCh Entrance Examination, C.S.M. Medical University, Lucknow only on June 21, 2008 between 10AM to 4PM
For application on paper format candidates can download the application form from www.kgmcindia.edu or get the same by hand from the office of the Coordinator, All India DM/MCh Entrance Examination 2008, C.S.M. Medical University, Lucknow
Fill up the form, paste the photograph, attach demand draft and required testimonials and send to the Coordinator, All India DM/MCh Entrance Examination 2008, C.S.M. Medical University, Lucknow, so as to reach there by 4 PM of May 30, 2008. No application will be accepted after this date.
Required testimonials
One passport size photograph (sharp) full front face, without cap/ hat/ dark glasses. If in the opinion of the Coordinator or the Chief Invigilator, the photograph of the Candidate is not recognizable or does not match with the candidate appearing in the examination the candidature of the student will be cancelled
Attested photocopy of High School Certificate
Attested photocopies of MBBS Degree and internship completion certificate
Attested photocopies of MD or MS Degree or Certificate from the Principal/ Dean/ Director/ Registrar of the institution from where candidate has passed MD/MS examination, specifically mentioning the subject and the year of passing the examination.
Attested photocopy of the permanent registration certificate
If employed, no objection certificate on prescribed format from the employer
